Routine Visits

Routine office visits are scheduled in advance. Since our schedule allows us to book appointments two to three months in advance, you will often be able to make the appointment as you leave the office. If not, please call us at least 4 weeks ahead of time to get an appointment.

We will try to remind you by phone about your appointment approximately 24 hours in advance. If you are unable to keep the appointment, please call us as soon as possible. There will be a charge of $25 for appointments which are not cancelled, unless there is a valid reason.

We try to keep the office running smoothly, but emergencies, late arrivals and unscheduled appointments may delay our normal schedule. Please realize that we always try to give each patient the necessary time, so your understanding of unexpected delays is appreciated.

Newborn check-up; Hepatitis B vaccine
Optional visit; Hepatitis B if not previously given.

2 months Check-up; DaPT, HIB, IPV, Hepatitis B, Prevnar.
4 months Check-up; DaPT, HIB, IPV, Prevnar.
6 months Check-up; DaPT, HIB, Hepatitis B, Prevnar.
9 months Check-up; Hematocrit, Prevnar.
12 months Check-up; chickenpox vaccine (Varivax), MMR, IPV.
15 months Check-up; DaPT/ HIB (combined).
18 months Check-up.
24 months Annual check-up.
3 years Annual check-up; hematocrit, PPD.
4 years Annual check-up; pre-kindergarten vaccines may be given at this time.
5 years Pre-kindergarten checkup; DaPT, IPV, MMR #2; may include hematocrit, urine, cholesterol, vision screening; completion of school forms; completion of hepatitis vaccines if not previously given.
6-10 years Annual check-ups.
11-12 years Check-up prior to entering middle school;; tetanus booster (dT) and Menactra before 7th grade; may include hematocrit, urine, vision screening; school and team sports forms completed. Hepatitis B vaccine if not previously given.
13-18 years Check-up prior to entering high school; Hepatitis B vaccine if not previously given; may include hematocrit, urine, vision screening; school, team sports and college forms completed.
